When do I complete the Student Responsibility form?
  1. When the student is turning in a missing or late assessment.  
  2. When the student knows he/she will be gone on the day an assessment is due.  

Student Responsibility Form and Request to Retake an Assessment will be accepted until 1 week before the end of each quarter.

When do I complete the Needing to Retake an Assessment  form?
  1. If you would like to request to improve on an assessment then complete the Needing to Retake an Assessment form.
